HEALTH chiefs are urging Powys residents who had their first dose of the Covid-19 vaccination on or before April 4, but have not yet had an invitation to attend a second appointment, to get in touch.
Powys Teaching Health Board (PTHB) are looking to fast-track second dose appointments as the vaccination programme in the county and throughout Wales continues to move at great pace.
If your first dose appointment took place at a mass vaccination centre on or before April 4 and you have not yet received your invitation to a second dose appointment then use the form at the following address, so that we can arrange this for you as soon as possible – visit https://pthb.nhs.wales/coronavirus/covid-19-vaccination/second-dose-appointments/second-dose-priority-access-form/.
Only complete the form if all the following criteria apply:
-You received your first dose of Pfizer/BioNTech or AstraZeneca on or before the date specified above
-You received your first dose at a mass vaccination centre in Powys or you are a care home worker/frontline care worker who received their first dose through an outreach programme (eg in a care home) or you have moved to Powys since your first dose
This form is not for people who received their first dose via a GP practice: contact your GP practice about your second dose appointment.
If the form is not visible below you can also access it via https://forms.office.com/r/VAfS5PB7SZ.
Meanwhile, over 103,100 people have now received their first dose from the Powys Covid-19 vaccination programme, and 70,900 people have also received their second dose.
PTHB have now delivered over 174,000 total doses through the Covid vaccination programme.
A total of 97 per cent of people in Priority Groups 1-9 in Powys have received their first dose, and over three quarters (87 per cent) have received double-dose protection.
Nine in ten (91 per cent) of the adult population in the county have received their first dose, and nearly two thirds (63 per cent) have received double-dose protection.
Last week (week ending June 1) saw over 7,100 vaccination doses delivered in the county.
