The two-vehicle crash happened on the B4385 between Newbridge-on-Wye and Beulah on July 21.
The road was closed for more than two hours following the collision involving a Suzuki motorbike and a white Ford Fiesta.
A Dyfed-Powys Police spokesperson said the motorbike rider was airlifted to hospital with "serious but not life threatening injuries".
